Public Health Leicester - healthy weight online survey
Overview
This survey is open to all residents in Leicester aged 18 years and older. We would welcome your thoughts about weight and the type of language used when discussed.
We recognise that weight can be a very senstive subject when referred to in any type of conversation or setting. If there is anything in the survey that needs further clarification or you find uncomfortable, please access the support links at the end of the survey.
There are two main parts to the survey and it should only take you 5-10 minutes to complete.
As an added incentive, If you would like to be entered into a draw to win a £20 Leicester Gift Card please enter your details at the end of the survey.
